What is Connecting Care?

The Tasmanian Anticipatory Care Project has now concluded. Connecting Care leaves two legacies for the future –

1. The Connecting Care Portal including the expanded Local Resource Directory (link) which we plan to gift to Central Coast Council by the end of the year, and
2. The Central Coast Community Wellbeing Governance model (link to infographic) for a healthy, educated and connected communtiy which is being implemented in partnership with Collective ed Ulverstone.

Connecting Care’s chronic health focus will continue along with Collective ed’s focus on educational attainment and pathways to employment – both are important components of community wellbeing.
